Adeosun Agnes Oluwatoyin (Mrs)-Chief Medical Record

Adeosun Agnes Oluwatoyin (Mrs), a Grade Level 14 officer is the Chief Medical Record, Health Service Commission.

She was born at Iropora, Irepodun Ifelodun Local Government Area of Ekiti State. A Christian by birth, Mrs Adeosun lost her Husband years back and has remained faithful to her children all through the years

Young Oluwatoyin began her academic progress at Our Lady of Fatima Primary School, Surulere and later, Lagos State College, Isolo for the Compulsory Secondary education. She immediately, after the Secondary School, enrolled with the School of Health Information Management, Lagos University Teaching Hospital at  thetertiary level, to obtain a Higher National Diploma (HND) in Health Information Management.

She observed her National Youth Service Corp Programme at Kogi State from where she came to Lagos to become a staff of the Lagos State Government in 1995.

She is currently a Member of Association of Health Records and Information Management Practitioner of Nigeria.

Since joining the Service through the Lagos State Health Service Commission, she had been posted to various hospital units as Medical Record Officer, before becoming the Chief Medical Record Officer with the Commission. The Hospital unit are namely:

General Hospital Isolo,(1995-1998) Apapa General Hospital (1999-2007) Orile-Agege General Hospital(2007-2013) Alimosho General Hospital(2013-2017) and General Hospital Lagos(2017-2019) before her posting to the Health Service Commission on 22nd December, 2019where she remains till date.

Her hobbies are Reading and listening to Gospel Music.
